  As the RN Case Manager, you will be responsible for delivering high-quality care to our Hospice patients, while continuously evaluating your performance and making necessary changes to improve productivity and quality of care.
  • You will make initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
  • You will identify patients' physical, psychosocial, and environmental needs and reassess as needed, no less than every 15 days.
  • You will initiate and coordinate the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition.
  • You will document problems, appropriate goals, interventions, and patient/family response to hospice care.
  • You will collaborate with patients, families, attending physicians, and other members of the IDG in providing patient and family care daily.
    Requirements
    • One year of clinical experience.Licenses and Certifications
      • Current RN licensure in the state of practice and one year of clinical experience.
      • Current CPR Certification.
      • Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.Additional State-Specific Requirements
        • VA: Experience with terminally ill required.
