the Position TravelNurseSource is working with Job.com to find a
qualified Hospice RN in Greensboro, North Carolina, 27405! Conduct
initial and ongoing assessments, monitor symptoms, and adjust the
plan of care as needed . Case manage a dedicated caseload, acting
as primary RN and main point of contact for patients and families.
Coordinate with providers, social work, chaplains, and other team
members to ensure holistic hospice care. Document all visits,
interventions, and changes in condition using HCHB (Homecare
Homebase) EMR?. Educate patients and caregivers on medications,
symptom management, and end-of-life care expectations 5 Benefits of
