Sentara Halifax is now seeking a Community Health Worker in South
Boston, VA!
The Community Health Worker works on the mobile care RV and travels
to various locations across the area. Hours may flex and some
weekends may be required.
In this position, you will w ork in collaboration with the
Continuum Case Manager and other healthcare team members to provide
comprehensive and coordinated care, provide education and behavior
modification strategies and provide social support to patients and
families. The Community Health Worker also acts as a patient
advocate and healthcare navigator in assisting with resource
allocation and management.

Job Summary
Works in collaboration with the Continuum Case Manager and other
healthcare team members to provide comprehensive and coordinated
care. Engages in interventions to improve self-management skills
and improve quality of life for patients with chronic and/or
complex illness. Provides education and behavior modification
strategies and provides social support to patients and families.
Acts as a patient advocate and healthcare navigator in assisting
with resource allocation and management.
Bilingual in English and Spanish is preferred. Experience working
in a community-based setting preferred. Knowledge of medical
terminology preferred
Qualifications:
HS - High School Grad or Equivalent (Required)
Driver's License - North Carolina, Driver's License -
Other/National, Driver's License - Virginia
