Job Description Job Description MUST READ DESCRIPTION. MUST Reside in the geographical area. Can be full time or part-time. 2 open Case Management positions to serve Durham, Wake, and Orange Counties. Position is mostly remote with occasional in office requirements. This position serves medically fragile children and disabled adults with the NC CAP/C Program and CAP/DA program. Candidate must have experience with case management, home and community services, experience with children and or adults with medical complexities and or physical disabilities. Must be able to make home visits as required. Must successfully pass a background check that includes an abuse registry check. Must have a Bachelor's Degree in SW from an accredited school of social work; Bachelors degree in a human services or equivalent field from an accredited college or university; a Bachelors Degree in a non human services field from an accredited college or university with two or more years of community experience in the health or medical field directly related to home care or long-term care; or be a licensed RN with case management experience.
